PUPPY QUESTIONAIRE
The following questions are being asked of you to determine if a Harmony Miniature Poodle would be the best choice of dog for you. The answers help us make the best choices we can for our puppies. Their life-long wellbeing is our primary concern. The more information you give us the better we can determine if we want to move forward with your applicaion. Completing this questionnaire is the first step in a screening process and is not a contract for a puppy.

- Please tell us about you and where you live. This should include who you live with (people and pets), type of home you have and at least the city and state in which you live.
- Please describe your perfect dog. What things are non-negotables and what things are you flexible about?
- Please give us contact information for three people who can vouch for as good pet owners. Ideally, these people will include your vet, groomer, trainer or person who referred you to Harmony Miniature Poodles. Please notify these people that we will be contacting them and give them permission to chat with us about you.
- What would a typical day look like for you and your Poodle? How do you see that changing over time?
- Tell us about your previous pets. What did you like best about them? Any things you'd change?
- What makes you the ideal owner for a Harmony Miniature Poodle?
